Pet Plans - Part IV

1. Type of Plan: Accident and Illness.
2. Annual Limits: $10,000 or unlimited. 
3. Reimbursements: 70%, 80% or 90% of covered vet bills. 
4. Deductibles: Annual Deductible with various pricing options. 
5. Eligibility: All Pets Ages 8 Wks. & Up Are Eligible to Enroll. However, certain restrictions on health conditions. 

Coverages: 

• Hereditary & Congenital Conditions covered. 
• Chronic Conditions covered. 
• Preventable diseases not covered. 
• Dental Issues covered. (Dental exam & cleaning). 
• Sick Visit Exam Fees covered with optional monthly fee.
•  Diagnostics, including Radiology & Lab Tests covered.  
• Surgery, Emergency Services & Hospitalization covered.  
• Specialized Care covered. 

1. Type of Plan: Accident and Illness. Also Accident only. 
2. Annual Limit: Annual limit of $2,500, $4,000, $5,000, $7,000, $10,000 and unlimited. 
3. Reimbursements: 70%, 80% or 90% of covered vet bills. 
4. Deductibles: Annual deducible of $100, $250, $500, $750 or $1,000. 
5. Eligibility: All Pets Ages 8 Wks. & Up Are Eligible to Enroll. No preventative care requirement before a claim is paid. No upper age limit. No clinical exam required for enrollment. 

Coverages:  

• Use any vet.  
• No annual wellness exam, dental exam or vaccine requirements.   
• Hereditary & Congenital Diseases covered (Bilateral exclusions include ligament and knee conditions).  
• Sick visit exam fees covered.   
• Preventable Illnesses covered.  Covers curable pre-existing conditions that are free from treatment and symptoms for 180 days 
• Dental Issues covered  Extraction only. (does not cover endodontic or orthodontic dental services such as caps, crowns, crown amputation, fillings, implants or root canals). 
• Chronic conditions covered.  
• Diagnostics, including Radiology & Lab Tests covered. Surgery, Emergency  Services & Hospitalization covered.  
• Specialized Care covered.

3. Reimbursements: 90% of covered vet bills, excluding exam fees. 
4. Deductible: Per condition deductible.  
5. Eligibility: Upper age limit of 14 years. 

Coverages:  

• Exam fees not covered.  
• Dental illnesses covered (annual dental exam required).  
• No wellness or preventative care coverage offered.  
• Hereditary & Congenital Conditions covered. (No bilateral exclusions for CCL injuries). 
• Chronic conditions covered. 
• Preventable diseases covered only if the illness occurs following vaccine and preventive care recommendations.  
• Diagnostics, including Radiology & Lab Tests covered.  
• Surgery, Emergency Services & Hospitalization covered.  
• Specialized Care covered.

2. Annual Limits: Note: Lifetime limit of $100,000 and $10,000 per incident limit 
3. Reimbursements: 100% of covered vet bills. 
4. Deductibles: Annual Deductible with various pricing options. 
5. Eligibility: All Pets Ages 8 Wks. & Up Are Eligible to Enroll. 

Coverages: 

• Hereditary & Congenital Conditions covered. (pets age 6 or older are not eligible for hip dysplasia coverage). 
• Chronic Conditions covered. 
• Preventable diseases covered. (Requires annual health checkup, annual dental exam, meds or treatment as required and regular vaccinations). 
• Treatments Resulting from Not Following Your Vet’s Advised or Required Preventive Care Measures not covered. 
• Limit for “anesthetic removals of ingested foreign bodies” in a year. 
• Extended waiting period for knee injuries, which is 180 days, and an extended waiting period for cancer, which is 30 days. 
• Dental Issues covered. (Dental surgery is not covered). 
• Sick Visit Exam Fees covered with optional monthly fee. 
• Diagnostics, including Radiology & Lab Tests covered.  
• Surgery, 
• Emergency Services & Hospitalization covered.  
• Specialized Care covered.
