Pet Plans - Part I

1. Type of Plan: Accident and Illness..  
2. Limits: Different annual plan limits ($2,500 to unlimited). 
3. Reimbursements: 70%, 80%, or 90% of reasonable and customary vet charges.  
4. Deductibles: Annual Deductible with various premium and deductible options.  
5. Eligibility: Pets ages 9 & up are not eligible to enroll in illness coverage. Health restrictions may apply.  

Coverages: 

• Hereditary & Congenital Conditions covered (Optional add-on for an additional monthly fee for pets ages 2 & under).  
• Chronic conditions covered (Optional add-on for an additional monthly fee for pets ages 2 & under).  
• Dental issues not covered 
• Recurring Conditions from Repetitive Occurrences covered. (Does not apply to ligament and knee conditions 
• Preventable illnesses covered, but limited (certain diseases are not covered unless they develop following vaccine).  
• Parasites & Illnesses They Cause not covered Sick visit exam fees covered (Optional for an additional monthly fee).  
• Diagnostics, including Radiology & Lab Tests covered.  
• Surgery, Emergency Services & Hospitalization covered.  
• Specialized Care covered.

3. Reimbursements: 70% to 90% of covered vet bills.  
4. Deductibles: Annual Deductible with various premium and deductible options.  
5. Eligibility: All pets are eligible to enroll, regardless of health, but age restrictions may apply.  

Coverages: 

• Hereditary & Congenital Conditions covered.  
• Chronic conditions covered.  Dental issues covered (only certain states and up to $1,000).  
• Preventable illnesses covered, but limited (certain diseases are not covered unless they develop following vaccine).  
• Sick visit exam fees covered.  
• Diagnostics, including Radiology & Lab Tests covered.  Surgery, 
• Emergency Services & Hospitalization covered.  
• Specialized Care covered.

• Preventable diseases covered only if the illness occurs following your  vaccine and preventive care recommendations. 
• Dental Issues covered. (Limited,  vet-administered teeth cleaning under general anesthesia is required for pets ages 3+ in the 13 months before service on periodontal disease claim. 
• Sick Visit Exam Fees covered with optional monthly fee. 
• Diagnostics, including Radiology & Lab Tests covered.  
• Surgery, Emergency Services & Hospitalization covered.  
• Specialized Care covered.

• Diagnostics, including Radiology & Lab Tests covered.  
• Surgery, Emergency Services & Hospitalization covered.  
• Preventable illnesses covered, but limited (parasites not covered. Plans do not cover any parasite infection where there is a preventive treatment available). 
• Specialized Care covered.
